The iPredict^TM app and web portal (portal.mhealth.astrazeneca.com/#/app/ipredict) have been developed by AstraZeneca UK (a private limited company registered in England and Wales under company number 03674842 with registered offices at 1 Francis Crick Avenue, Cambridge Biomedical Campus, Cambridge, CB2 0AA), who is represented in United States with offices at Wilmington (1800 Concord Pike, Wilmington, DE), Gaithersburg (950 Wind River Ln, MD) and Waltham (35 Gatehouse Dr, Waltham, MA) and with the assistance of trusted third party vendors, who provide and support the technology.

• The app and web portal will collect certain personal data, which will be provided by you or your healthcare professional (e.g.: contact details and medical data).
• The main purpose of the iPredict^TM programme is to develop a predictive algorithm that will in the future help Asthma patients predict changes in their condition. As such, the app will assemble information about your condition and your medication. The iPredict+^TM programme is also designed to help AstraZeneca and its partners improve the iPredict^TM services and for scientific research purposes.
• Your personal data will be retained only as necessary to deliver this service to you and to comply with applicable laws. Only your healthcare professional practice and minimum IT support personnel from both AstraZeneca and its trusted third parties will have access to data that personally identifies you. Other parties will only have access to coded/anonymised data so that you cannot be personally identified.
• All personally identifiable information is stored on secure servers in the EU (Ireland) and managed by AstraZeneca. Coded/anonymous data from your device is stored with the manufacturer of the device in US and NZ. This data is used to improve the product.
• You can stop using the app at any time by telling your healthcare professional and uninstalling the app from your phone. To comply with data security protocols that require traceability to safeguard that only authorized personnel have had access to your personal data when you were using the app, data entry records will be retained in an encrypted manner.
