The Turbu+TM Insights Service is provided by AstraZeneca (a private limited company registered in England and Wales under company number 03674842 and with registered offices at 1 Francis Crick Avenue, Cambridge Biomedical Campus, Cambridge, CB2 0AA). The term 'Service' refers to both the Turbu+TM Insights portal and the Turbu+TM Insights app. The Service is supported by trusted third party vendors, who provide and support the technology. We take the privacy of our users and the security of their personal information very seriously.
Personal data entered into the Service is provided by you or your Site Investigator in relation to your personal details (such as name, email address, phone number, gender, etc.), your health status, medication and health related goals. The Service will track medication intake, including how you take your medication and may also ask you to track your symptoms. Where requested, permission to use your location is required only to support the Bluetooth pairing of a mobile phone with the Turbu+ Device. The app does not perform any activity in the background with regards to the location usage. You will also be asked to complete some optional questionnaires in order to help us tailor the solution to your specific needs.
The main purpose of the Turbu+TM Insights Service is to help you manage your condition in conjunction with your Site Investigator and properly take your medication. The Service will capture the set regimen and medication use data. Based on the specific patient medication use pattern (adherence data), the patient receives regular medication reminders and missed-medication messages. The Service will also help AstraZeneca and its partners to improve the Turbu+TM Insights Service. It is also designed to help AstraZeneca and its partners to conduct scientific research, which may help us develop better drugs or medical aids.
Your data may be shared with certain third parties such as: IT software providers involved in the development and maintenance of this application; auditors and consultants to verify our compliance with external and internal requirements; statutory bodies, law enforcement agencies and litigants, as per a legal reporting requirement or claim; and a successor or business partner to an AstraZeneca group company in the event that it sells, divests or sets up a collaboration/joint venture for all or part of its business.
Only your Site Investigator practice and minimum IT support personnel from both AstraZeneca and its trusted third parties will have access to data that personally identifies you. Your healthcare professional may download your personal data from the application into his/her own recordkeeping system for the purposes of helping you manage your condition: your healthcare professional will be responsible for using this information in accordance with all applicable laws and regulations. Other parties will only have access to coded data so that you cannot be personally identified.
Your personal data may be cross checked by your Site Investigator with the data held on your Site Investigator's system and with you to ensure accuracy. Your Site Investigator will have the ability to update your medical information based on any changes to your condition. Any changes are tracked within the Service.
All personally identifiable information is stored on secure servers in the EU (Ireland) and managed by AstraZeneca. Coded/anonymous data from your device is stored with the manufacturer of the device in the US and New Zealand. This data is used to improve the product.
You will have the ability to see most of your personal information directly in the Service yourself. You can also update your personal contact details (e.g. email address, phone number, portal password) through the Service. You may contact AstraZeneca at www.astrazenecapersonaldataretention.com at any time to request access to the personal data we hold about you, to correct any mistakes or to request deletion of the same or withdraw your consent to certain types of processing of your personal data. We will endeavor to comply with such request within one month of receipt of the request. If such a request places AstraZeneca or its affiliates in breach of its obligations under applicable laws, regulations or codes of practice, then AstraZeneca may not be able to comply with your request but you may still be able to request that we block the use of your personal information for further processing. You may also have a right to data portability to another Data Controller under certain circumstances.
If you decide to leave the Service, your access will be removed so you will be unable to use the Service from the point at which the Service confirms you've been withdrawn. You can stop using the Service at any time by telling your Site Investigator and uninstalling the Turbu+TM Insights app from your phone. Information that would personally identify you would then be removed within a maximum of 4 weeks. To comply with data security protocols that require traceability to safeguard that only authorized personnel have had access to your personal data when you were using the Service, data entry records will be retained in an encrypted manner for one year.
For scientific research purposes information about you would be retained such as medication intake, how you take your medication, response to questionnaires in accordance with AstraZeneca's document retention standards, for a period of up to 15 years.
